Aynı dönemde komplikasyon gelişmeyen ve ardışık ameliyat edilen 215 hasta kontrol grubu olarak tanımlandı ve risk faktörleri değerlendirildi. Bulgular: Yaş ortalaması 58.9 ± 12.1 yıl (18-83 yıl) olan hastaların çoğunluğu erkekti (% 82.0, n = 278). Çalışmamızda 65 yaş üzeri, erkek cinsiyet, kronik kalp yetmezliği(KKY), koroner arter hastalığı(KAH) ve kronik obstruktif akciğer hastalığı(KOAH) varlığı, pnömonektomi, intraoperatif kan ürünleri kullanımı, kanamaya bağlı yeniden operasyon, operasyon süresi> 4 saat ve peroperatif inotrop kullanımının PC gelişimi açısından istatistiksel açıdan anlamlı olarak görüldü. Our goal is to identify SSIs associated risk factors and develop a predictive model. Methods: A total of 2521 patients who underwent surgery, between June 2018 and May 2019, in four surgery departments, at the Taza Provincial Hospital (Morocco) were diagnosed for SSI according to the standards of Centers for Disease Control and Prevention. Then, the SSIs’ risk factors were assessed by uni-variate statistical analysis and logistic regression using Scikit Learn of Python. Results: The average age of the studied population is 35 years (±1). The overall SSIs incidence is 6.3% (distributed between 17.95%, 6.86%, 6.67% and 3.16% respectively in child, female, male and gynaecological-obstetrical surgeries). The univariate statistical analysis has shown a high significance (p < 0.001) and a very significant (p < 0.01) relationships for almost all risk factors and the logistic regression model has revealed a strong association between SSI and people who have undergone surgery in the last months before surgery, urinary catheter, the antibiotic use duration, co-morbidity, ASA score, duration of intervention, emergency preoperative and postoperative durations, service, specialty and age rang which remain highly significant variables for SSI, the prediction score exceeds 96% which justifies our model’squality. Conclusion: SSIs are generally frequent among postoperative patients; and their extent was acceptable in some services and high in others. Therefore, Pre-operative preparation, post-operative surveillance and the wards environment quality are necessary to reduce SSIs rates in hospital. -- The Journal of Infection in Developing Countries (Temmuz 2021)

  • Dear Fazlı YANIK, We hope that you share your valuable opinions regarding the publication of the work titled 860028 - "Uzamış Hava Kaçağı Ve Heimlich Valf" in our journal. Article abstract: Amaç: Değişik nedenlerden dolayı akciğer ameliyatı yapılmış ve ameliyat sonrası uzamış hava kaçağı gelişen hastalarda Heimlich valf’in etkinliği araştırıldı. Gereç Ve Yöntem: Haziran 2014 - Aralık 2018 tarihleri arasında kliniğimizde değişik nedenlerden dolayı ameliyat edilen ve uzamış hava kaçağı sorunu gelişen 33 hastada mevcut göğüs tüpünden HV uygulandı. Taburculuk sonrası Heimlich valf’in etkin kullanılıp kullanılmadığını değerlendirmek için en erken hastalar üçüncü gün kontrole çağrıldı. Kontrollerde hava kaçaklarının varlığını değerlendirmek için içerisine 50 cc serum fizyolojik konulmuş plastik kaplarla kaçak testi yapıldı. Bulgular: Olguların 22’si (%66,7) erkek, 11’i (%33,3) kadın olup, ortalama yaş 50,3 (20-72) yıl olarak tespit edildi. Otuz üç hastanın 13’ü büllöz akciğer hastalığı, dokuzu diffüz interstisyel akciğer hastalığı, dördü akciğer kanseri, dördü ampiyem, ikisi nodüler akciğer hastalığı ve biri kist hidatik nedeni ile ameliyat edildi. Cerrahi olarak en sık uygulanan işlemler kama rezeksiyonu ve bül eksizyonuydu. Ameliyat sonrası Heimlich valf’e alınma süresi ortalama 10,2 gün (dağılım 7-15 gün) olarak tespit edildi. İki (%6,1) olgumuzda işlem başarısız kabul edildi. İşlemin başarı oranı %93,9 olarak tespit edildi. Sonuç: Heimlich valf’ın akciğer ameliyatlarından sonra uzamış hava kaçaklarının tedavisinde, hastanın taburculuk işlemini hızlandırması ve yaşam kalitesini artırması nedeni ile etkili bir yöntem olduğunu düşünmekteyiz. Article abstract: Amaç: Bronkoskopi boğulma hissi ve öksürük oluşturan invaziv bir işlem olduğundan kişilerde korku, rahatsızlık hissi ve kaygıya yol açabilmektedir. Bu araştırmanın amacı, bronkoskopi uygulanan hastalarda işlem öncesi korku varlığını ve korku nedenlerini belirlemektir. Araçlar ve Yöntem: Tanımlayıcı tipteki bu araştırma Nisan 2019 – Eylül 2019 tarihleri arasında bir üniversite hastanesinde çeşitli endikasyonlarla elektif bronkoskopi uygulanan, araştırmaya katılmaya gönüllü olan, 18 yaşını dolduran 138 hastanın katılımıyla gerçekleştirildi. Araştırma öncesinde etik kurul ve kurum izinleri alındı. Ayrıca hastalar araştırma hakkında bilgilendirilerek yazılı izinleri alındı. Veri toplamada “Hasta Tanıtım Formu” ve “Bronkoskopi Korku Anketi” kullanıldı. Veriler SPSS 22.0 programında ve Ki kare testi kullanılarak analiz edildi. p<0.05 istatistiksel olarak anlamlı kabul edildi. Bulgular: Hastaların yaş ortalamasının 62.4±11.5 (24-88) yıl, %79,7’sinin (n=110) erkek, %78.3’ünün (n=108) ilköğretim mezunu olduğu belirlendi. Hastaların %93.5’inin (n=129) işlemi gerektiğinde aynı şartlarda tekrar yaptırmayı kabul ettiği saptandı. Hastaların bronkoskopi sürecinden korkma durumlarının yaş ve cinsiyet göre istatistiksel açıdan anlamlı derecede farklılık gösterdiği belirlendi (p<0.05). Hastaların % 57,2’sinin (n=79) genel olarak bronkoskopi sürecinden korktuğu belirlendi. Sonuç: Bu araştırma sonuçları, bronkoskopi uygulanan hastaların genel olarak sürece ilişkin korku yaşadıkları belirlendi. Saygılarımızla, Dergi Editörlüğü dergi@tkdcd.org Makale Başlığı: Cerrahi Alan Enfeksiyonlarının Azaltılmasında Klorheksidin Glukonatın Rolü: Randomize Kontrollü Çalışma Makale Özeti: Amaç: Bu çalışma ameliyat öncesi Klorheksidin Glukonat (KG) ile yapılan cilt temizliğinin cerrahi alan enfeksiyonlarının ve bakteri kolonizasyonunun azaltılmasındaki rolünü belirlemek amacı ile yapıldı. Çalışma Planı: Bu çalışma bir devlet hastanesinde 61'i müdahale grubunda, 59'u kontrol grubunda olmak üzere 120 hasta üzerinde gerçekleştirildi. Müdahale grubu KG içerikli duş jeli kullanırken kontrol grubu antiseptik ürün içermeyen duş jeli/ sabun kullandı. Tüm hastalardan banyo önce ve sonrasında ameliyat bölgesinden kültür çubuğu ile kültür alındı. Hastalar taburcu olana kadar her gün taburcu olduktan sonra ameliyatın 10. ve 30. günlerinde lokal olarak ateş, kızarıklık, ödem ve akıntı yönünden takip edildi. Hastaların ateşi, nabzı ve kan basıncı da takip edildi. Bu veriler kaydedildi ve istatistiksel olarak analiz edildi. Bulgular: Çalışmaya dahil edilen 120 hastanın % 3.3ünde, kontrol grubu 59 hastanın %6.8inde enfeksiyon tesbit edildi. Müdahale grubunda hiç enfeksiyona rastlanmadı. Bakteri kolonizasyonu açısından incelenen 5 bakteriden Koagülaz negatif stafilokokların (KNS) üremesindeki azalma istatistiksel olarak anlamlı bulundu. Sonuç: Bulgular, torasik cerrahi ameliyatı öncesi KG ile cilt temizliğinin, SSI'yi önlemede ve KNS kolonizasyonunu azaltmada faydalı olabileceğini göstermektedir. (Eylül 2020)
